Three Pathways Community HUBs Earn Certification & Re-Certification

The Pathways Community HUB Institute® (PCHI®) is recognizing three Pathways Community HUBs (PCHs) that have recently achieved certification milestones—reflecting the strength of their work in supporting individuals with complex needs and building coordinated care networks in their communities.


Newly Certified

Health Care Access Now (Cincinnati, Ohio) has earned initial PCHI Certification for a two-year period. Through its work, HCAN utilizes certified community health workers (CHWs) to provide one-on-one support, helping individuals navigate healthcare systems, access consistent medical care, and address barriers that impact overall well-being. Its commitment to training CHWs and delivering coordinated services plays a critical role in improving outcomes across the region.


Re-Certified for Three Years

Two established HUBs have also achieved re-certification, demonstrating sustained performance and continued adherence to the PCHI® Model:


In Cuyahoga County, the Better Health Pathways HUB continues to grow its network and reach. By the end of 2025, the HUB included 15 care coordination agencies and 75 Community Health Workers and supervisors, supporting 5,400 individuals and addressing more than 30,700 needs since 2020. In 2025 alone, enrollment reached 1,654 individuals, alongside a 70% increase in referrals—reflecting the trust partners place in the HUB to connect people to support.


In Houston, the Harris County Pathways Community HUB continues to demonstrate strong outcomes through its network of CHWs and partners. In the past year alone, the HUB successfully completed more than 1,000 pathways—with 84% of all opened pathways reaching completion—and supported positive outcomes for families, including 88% of babies born at a healthy birth weight.


Re-certification re-affirms that these HUBs are consistently delivering measurable results, supporting individuals through proven pathways, and maintaining the integrity of the Model in their day-to-day work.
