Wrapping Up 2025: Final Certifications and Recertifications of the Year

As we close out 2025, PCHI^® is proud to announce our final round of certifications and recertifications for the year.

One Pathways Community HUB has earned full certification for the first time, while two Pathways Agencies have successfully completed recertification, demonstrating their continued excellence in delivering outcome-based, whole-person care coordination.

Newly Certified Pathways Community HUB (Nov. 2025-Nov. 2027):

• Community Health Organization Improving Care and Equity (CHOICE) Regional Health Network – Olympia, Washington

Re-certified Pathways Agencies (Nov. 2025–Nov. 2028):

• Central Ohio Pathways Community HUB – Columbus, Ohio

• Community Connections HUB – Northern Michigan

Each of these organizations brings together partners across healthcare and social service sectors to help individuals and families navigate systems of care and access the support they need. Through the PCHI Model, they are tracking progress, addressing barriers, and tying results to payment, which creates a stronger, more responsive network of care.
