Three new Pathways Community HUBs Earn Certification, one Pathways Agency Re-certified

The Pathways Community HUB Institute® (PCHI®) is proud to announce that three new Pathways Community HUBs (PCHs) have achieved certification, and one Pathways Agency (PA) has successfully earned recertification. These organizations are strengthening support networks in their communities—connecting individuals to care, services, and resources that lead to measurable improvements in health and well-being.
Newly Certified Pathways Community HUBs (July 2025–July 2027):
· Community Health Connect PCH – Austin, TX
· Lorain PCH – Lorain, OH
· Southern Nevada PCH – Las Vegas, NV
Recertified Pathways Agency (July 2025-July 2027):
· Rural OB Access & Maternal Services (ROAMS) – Taos, NM
Since 2013, the PCHI Certification Program has helped ensure that Pathways Agencies and Pathways Community HUBs faithfully implement the PCHI® Model—a proven community-based care coordination framework. The Model unites local agencies, healthcare providers, and community-based organizations to identify and reduce barriers to better health. It tracks progress across medical, social, and behavioral domains and ties payments to successful outcomes.
