Study shows how the PCHI Model Reduces Low Birth Weights

Dr. Sarah Redding, Founder and CEO of PCHI, was lead author of a 2015 study about the importance of utilizing community health workers in the coordinating care for mothers to be, specifically how care coordination can address poor birth outcomes including low birth weight. Through the Community Health Access Project (CHAP) in Mansfield, CHWs identified women at risk of having poor birth outcomes, connected them to health and social services and tracked outcomes. What it showed was there was a 60% reduction in low birth weight births, and > $5 return for every $1 invested in Pathways HUB model. Read the study
