Aligning Systems for Community Health

Kent State University’s Center for Public Policy & Health, in collaboration with Akron Children’s Hospital, the Pathways Community HUB Institute® (PCHI®), and the Georgia Health Policy Center, conducted an assessment of the national certification standards and cross-sector coordination efforts in communities using the PCHI® Model.
