The Pathways Community HUB Institute® Model assists communities in creating a community-based care coordination network that identifies individuals with modifiable risk factors – medical, social, behavioral – and connects them to services, tracks outcomes and contracts with payers that directly tie payment back to those outcomes. The PCHI Model is the quality improvement framework for communities to build their own inclusive network of care coordination to align resources and achieve positive outcomes.
