CommonSpirit Health Partners with PCHI to Advance Health Equity

Since 2020, PCHI has proudly partnered with CommonSpirit Health, a leading national non-profit health system, in reimagining health across communities in the U.S. Together, we’re advancing health equity through a transformative journey to scale the PCHI® Model in communities where CommonSpirit provides services. This network approach connects under-resourced populations with essential health and social care services, leading to improved health outcomes.

 

Our collaboration also involves an innovative financing approach to support the establishment of Pathways Community HUBs (PCHs) in these communities. The PCHs are designed to address the social determinants of health and provide comprehensive care coordination by utilizing community health workers, aiming to improve health and quality of life for enrolled participants.

 

CommonSpirit's catalyst investment strategy, along with its local relationships, has raised over $13 million in external funding to build Pathways Community HUBs. This strategy provides seed and match-contingent funding as well as technical support to communities, fostering local ownership and sustainability. The goal is for these communities to develop fully operational PCH networks with healthcare payer contracts, which will provide the essential support for non-profits that typically struggle to access healthcare funding.

 

The PCHI Model is proving to be a transformative force, showcasing the power of intentional collaboration and the impact of community health workers in delivering whole-person care. In just 12 months of providing CHW services, the Southern Nevada PCH, led by Comagine Health, was able to  decrease rates of preterm birth and low birth weight for high risk moms by 65 % and 50% respectively.

 

While there is still much to do, we’re excited about the work ahead and its potential to improve the health and well-being of our communities. We will continue to share insights and learnings as we move forward. By partnering with health systems, we’re able to transform the lives of individuals and families.
