Implementing a countywide approach, Stark County THRIVE Pathways HUB works to identify local causes of infant mortality through the use of accurate data, solid scientific analysis, and evidence-based interventions to implement programs to reduce Stark County's unacceptable disparity and infant mortality rates. Goal: All babies in Stark County will celebrate their first birthday. Long Term Measure: Decrease the overall, black, and white infant mortality rates to less than 6.0. Long Term Measure: Decrease the disparity rate ratio to 1.0. Data from pregnant and maternal clients enrolled in the Stark County THRIVE Pathways HUB is analyzed to evaluate the impact of the HUB and CHW on the following metrics: Reduce very preterm births <32 weeks gestation Reduce preterm births <37 weeks gestation Reduce very low birth weight births (<1,500g) Reduce low birth weight births (<2,500g) Moms with normal BMI prior to pregnancy Moms who didn't smoke prior to pregnancy Moms who didn't smoke at any time during pregnancy Increase number of pregnant women obtaining early entry into prenatal care by more effectively and efficiently engaging with pregnant women at-risk for poor birth outcomes with focus on African American women. Increase the number of fathers served by CHW to strengthen families Improve chronic conditions of adults enrolled in the Stark County THRIVE Pathways HUB
