Community Action Pathways HUB

The Community Action Pathways HUB focuses on the empowerment of at-risk individuals to overcome the social determinants of health and racial inequities in order to elevate the health of Stark County. The CAPHUB strives to challenge systematic barriers and ignite change that ensures that everyone has access to the services, tools, and resources necessary to promote good health and wellbeing.
