Better Health Pathways HUB

Better Health Partnership(BHP) is a regional health improvement collaborative that brings health care and other sectors together to share best practices and accelerate data-informed improvements inequitable population and community health. In 2020, we launched a Pathways Community HUB; an evidence-based care coordination model that utilizes Community Health Workers to connect at-risk individuals to care and resources to help meet health, social and economic needs.
